Factors associated with the incidence of post-pancreatectomy diabetes: a nationwide population-based cohort study

Background: Limited information was available on factors associated with post-pancreatectomy diabetes. The purpose of this study is to investigate the one-year incidence of diabetes after pancreatectomy and associated factors.Methods: Using insurance claims from the Taiwan's National Health Insurance Research Database, we conducted a nationwide population-based cohort study consisted of 4236 patients aged 18 years and above without diabetes receiving pancreatectomy. The new-onset diabetes after pancreatectomy within one year was identified. Perioperative sociodemographics, 12 coexisting medical conditions and types of pancreatectomy were considered as potential factors associated with post-pancreatectomy diabetes in the multivariate logistic regressions with calculating adjusted odds ratios (ORs) and 95% confidence intervals (CIs).Results: The one-year incidence of diabetes after pancreatectomy was 17.7%. Compared with patients aged 18-39 years, the older patients aged 60-69 years had increased risk of post- pancreatectomy diabetes (OR=1.87, 95% CI=1.36-2.58). Low income is one of factors associated with post- pancreatectomy diabetes (OR=1.67, 95% CI=1.01-2.76). Perioperative coexisting medical conditions were associated with diabetes after pancreatectomy, such as renal dialysis (OR=2.12, 95% CI=1.06-4.25), hyperlipidemia (OR=1.75, 95% CI=1.31-2.32), pancreatitis (OR=1.47, 95% CI=1.20-1.79), and hypertension (OR=1.20, 95% CI=1.01-1.44). Types of pancreatectomy were not predictors for post-pancreatectomy diabetes.Conclusion: This study investigated patients receiving pancreatectomy had high one-year incidence of diabetes after surgery, particularly for those with older age, renal dialysis, hyperlipidemia, pancreatitis and hypertension.
