A Factual Survey of Non-Cardiac Chest Pain (NCCP) in Japan

Objective
The most important cause of diseases resulting in chest pain is cardiac chest pain. A detailed examination of any heart disease often detects no abnormality, and these cases are called Non-Cardiac Chest Pain (NCCP). NCCP attracts less attention in Japan than in Europe and America, resulting in there being few reports on its actual condition and pathology. Therefore, this study aims to survey prevalence of NCCP and its disease characteristics in Japan through the implementation of a large-scale epidemiological survey.
Methods
A cross-sectional study of 10,000 male and female subjects of various age ranges from their 20s to 70s based on Japanese demographics was conducted by means of an online survey.
Results
There were 524 individuals with frequent or occasional symptoms of chest pain out of 10,000 individuals (5.3%). Among them, 194 (30%) individuals consulted with a hospital or clinic complaining of their symptom of chest pain, whereas 330 (70%) did not. Among the individuals who consulted with a hospital or clinic complaining of their symptom, 177 individuals (91%) were diagnosed with no heart disease, whereas 17 individuals (9%) were diagnosed with heart disease. We also found that 9.7% of individuals who consulted a hospital or clinic complaining of chest pain were transported to a hospital by an ambulance or consulted with a nighttime emergency room.
Conclusions
We are the first to report the prevalence of NCCP in a Japanese population. Our study found that the prevalence of NCCP in Japan was 5.3%, and 30% of NCCP patients visited a medical institute for consultation. Furthermore, about 10% of the medical examinees of NCCP visited an emergency room, suggesting the importance of NCCP from a medical as well as economical viewpoint.
