Real-world data of atherosclerotic cardiovascular diseases in patients with non-alcoholic fatty liver disease based on medical claim database in Japan

BACKGROUND :The most common cause of death in patients with NAFLD are reported to be cardiovascular disease, independent of other metabolic comorbidities. However, the degree to which NAFLD contributes to the incidence of atherosclerotic cardiovascular diseases (ASCVD) is unknown, because many metabolic factors coexist with NAFLD. To estimate the risk of new-onset of ASCVD in NAFLD patients, we analyzed real-world data using claim database in Japan.
METHODS :Population based, electronic primary healthcare database (2013-2019: Japan Medical Data Center [JMDC]) were used. The primary outcome was the new-onset of ASCVD in the NAFLD and non-NAFLD groups. Patients were followed at least for ≥12 months.
Results:A total of 450,000 were included in this study. Unadjusted incidence rate of ASCVD were higher in NAFLD patients compared with non-NAFLD patients. Rates of ASCVD were 1.2 and 0.5 per 1000 person years in patients with and without NAFLD, respectively. Compared with non-NAFLD, the hazard ratio of NAFLD [without high-triglyceride (TG) or diabetes mellitus (DM)], NAFLD with high-TG, NAFLD with DM, NAFLD with high-TG and DM are 1.2, 2.3, 2.6 and 3.3, respectively.
Conclusion:The risk of ASCVD increases in case of NAFLD complicated with high-TG and/or DM. It is imperative for physicians to recognize the multiple risk factors for ASCVD in NAFLD patients.
