Our ESD training system for Asian endoscopists

【Background】
Endoscopic submucosal dissection (ESD) is developed in Japan and widely used in Asia. However, an effective training system hasn’t been established in Asian countries. Therefore, we are providing ESD training system for foreign doctors.
【Training program】
Weekly lectures and ESD hands-on with the ex-vivo pig model. Furthermore, trainees who could stay over 4 months can experience ESD to the patient with a temporary medical license issued by the Ministry of Health, Labor and Welfare (MHLW).
【Aim&Method】
The aim is to evaluate our training system.
1) Questionnaire survey was done from 23 doctors who have trained over 1 month between 2016 and 2019. The review by a 5-point grading scale and their situation regarding ESD were asked.
2) Short-term outcomes of 23(Stomach20, Colon3) ESD cases performed by 4 Asian trainees in our hospital were evaluated retrospectively.
【Results】
1) 21/23 answers were obtained. The mean endoscopy experience years were 15.1 years. 16(76%) doctors have experienced ESD before. After training, 20(95%) doctors underwent ESD and 13(62%) became a supervisor of ESD in their hospitals. All the feedbacks were 4 or 5 points. 2) The mean size of resected sample size and procedure time (Stomach/Colon) were 34.0/35.7mm and 57.9/48.7min. En bloc plus R0 resection and Self-completions rate were 100% respectively. No adverse event occurred.
【Conclusion】
Our training could offer enough satisfaction and favorable results. As next step, we are organizing annual training course in their country.
