Ability of closure for post-endoscopic submucosal dissection perforation by hydrophobized microparticles (hMPs) wound dressing.

Highly adhesive gelatin hydrophobized microparticles (hMPs) are high tissue adhesion and angiogenic properties. They form a gel-like membrane with strong adhesion to ESD ulcers, covering the ulcer surface. Aim: To determine the perforation coverage effect of hMPs on the gastric and duodenal ESD perforation model. Methods:1)The porcine resected stomach was subjected to ESD, and a pinhole-shaped perforation was created. hMPs were divided into hMPs sprayed and non-sprayed groups. Air was injected into the stomach and pressurized to 30 mmHg, and air leaks were observed in water, and the pressure at the time of air leaks was measured.2)Duodenal ESD was performed on miniature swine model, and they were divided into two groups: those with and without artificial pinhole-shaped perforations, and those without hMPs sprayed. They were divided into hMPs sprayed and non-sprayed groups, respectively, and slaughtered on the third postoperative day, and the inflammation of intrinsic muscle layer and serosa was compared and examined. Results: 1)The pressure could not be measured in the non-sprayed group, whereas in the sprayed group, no air leak was observed when pressurized to 30 mmHg in 4 out of 5 cases. 2)The sprayed group suppressed inflammation of the intrinsic muscular layer and serosa in both cases compared to the non-sprayed group. Conclusion: hMPs is expected to be applicable to post-ESD perforation and prevent delayed perforation.
