Genetic mutation of DNA in intraoperative peritoneal washing lavage from pancreatic cancer patients intended to curative resection by Next-Generation Sequencing Young Award

Background: The efficacy of next-generation sequencing (NGS) of tumor-derived DNA extracted from intraoperative peritoneal washing lavage (IPWL) of patients with pancreatic ductal adenocarcinoma (PDAC) intended for curative resection has not been extensively investigated. Methods: This case series study included twelve patients who intended curative resection at our hospital between January 2020 and December 2021. The IPWL cytology (CY) was checked, and NGS for genomic tumor-derived DNA from IPWL was performed to check whether a genomic mutation in tumor-derived DNA can be detected from samples of patients with PDAC intended for curative resection Results: Actionable gene alterations were detected in 8 (80.0%) out of 10 patients with CY (-) from NGS of their IPWL. Among the patients with actionable gene mutations of DNA from IPWL, 3 (37.5%) had peritoneal dissemination after surgery within twelve months. The main genomic mutation from the NGS of IPWL was KRAS mutation (9 patients, 75.0%), followed by TP53 (3 patients, 25.0%) and SMAD4 (1 patient, 8.3%). Conclusions: Genomic mutation in tumor-derived DNA from IPWL of PDAC patients with CY (-) intended for curative resection using NGS may potentially have clinical utility as a predictive biomarker for early peritoneal dissemination recurrence.
