The efficacy of contrast-enhanced EUS for possible mural nodules in IPMN Young Award

The presence of mural nodule (MN) indicates malignant transformation of IPMN. It is difficult to distinguish true MN from mucus plugs (MP) by EUS alone. Contrast-enhanced (CE)-EUS has possibility to differentiate true MN from MP.
Aims: To evaluate the efficacy of CE-EUS for differentiation true MN from MP and carcinoma from adenoma.
Patients and methods: 101 patients who were diagnosed as IPMN with MN like structures by EUS and underwent CE-EUS were included. MN like structures without perfusion on CE-EUS were defined as MP and with perfusion were defined as true MNs.
Result: CE-EUS showed perfusion in 39 MNs and no perfusion in 62 MNs. Among 39 MNs with perfusion, 29 patients underwent surgical resection, although surveillance was performed in the remaining 10 patients. Among 62 MNs without perfusion, 4 patients underwent surgical resection because of the change in findings during follow-up. Surgical resection was performed in total of 33 patients and showed 23 carcinoma and 10 adenoma. Therefore, differentiation of true MN from MP was successful in all. Among 39 true MNs, the height of MN in carcinoma was significantly higher than that of adenoma.
Conclusion: CE-EUS was accurate procedure to differentiate true MNs from MPs.
