Current evidence and technical challenges for laparoscopic surgery for far advanced gastric cancer

In the era of early gastric cancer, laparoscopic gastrectomy (LG) had spread in Japan recently, not only high but small volume center. LG for advanced gastric cancer was still controversial because of unclear longtime survival. Some randomized clinical trials will answer this question, so, next important clinical question is validity of laparoscopic surgery for far advanced or complicated gastric cancer such as multiple or bulky lymph node cases, direct invasion to adjacent organ, bulky tumor, EGJ cancer. Discussion points will be technical challenges and long and short time results. Technical aspects will include laparoscopic or robotics, energy devise choice, tumor handling technique, approach style. Knocks and pit fall from experienced specialists with many cases would be helpful for next generation. Now, the time for discussing this core question had come.
