Endo-Hepatology: The Changing Paradigm of Endoscopy in Liver Diseases and Varices
Introduction
Endo-hepatology is revolutionizing the management of liver diseases and their complications. The integration of endoscopic techniques into hepatology practices offers diagnosing and treating portal hypertension, esophageal and gastric varices, and other liver-related conditions.
Non-alcoholic fatty liver disease (NAFLD)
Endo-hepatology introduces EUS-guided liver biopsy (EUS-LB) as a precise alternative diagnostic tool for NAFLD. Studies show that EUS-guided biopsy provides larger and more adequate tissue samples compared to traditional percutaneous approaches, enhancing diagnostic yield. Furthermore, EUS-LB allows obtaining liver tissue from both lobes of the liver. This technique is particularly advantageous in patients with obesity or those at increased risk for complications from percutaneous liver biopsy.
Portal Vein Pressure Measurement
EUS-guided portal pressure measurement is an emerging technique that offers an alternative to hepatic venous pressure gradient (HVPG) measurement. EUS allows direct pressure measurement of the portal vein and hepatic vein. This approach not only enhances diagnostic accuracy but also enables repeated measurements, which are crucial for monitoring disease progression and therapeutic response.
Variceal Bleeding Management
Endoscopic variceal ligation (EVL) is the cornerstone of variceal bleeding management. EUS-guided therapy is emerging as a promising tool, allowing direct visualization and targeted treatment of varices. EUS-guided coil embolization and cyanoacrylate injection are innovative methods that provide enhanced control of variceal bleeding, particularly in patients with gastric varices or those who have failed conventional treatments.
Conclusion
Endo-hepatology represents a paradigm shift in the management of liver diseases and varices. By merging endoscopic innovations with hepatology expertise, this field offers enhanced diagnostic accuracy and therapeutic options, ultimately improving patient care. Ongoing research and technological advancements will continue to shape the future of endo-hepatology.
