Current situation of Endo-Hepatology in Europe | |||

Endo-hepatology refers to the integration of advanced endoscopy within the practice of hepatology centred around endoscopic ultrasound (EUS). With the development of EUS-guided liver interventions over the past decade, interventions that can be performed now with EUS include liver biopsy, portal pressure gradient measurement, coil embolization of gastric varices, and evaluation of fibrosis (shear wave elastography). EUS-guided portal pressure measurement might become a conceivable alternative to hepatic vein catheterisation since it holds the potential of broader applicability given the wide accessibility of EUS in everyday gastrointestinal practice. EUS-guided liver biopsy is a well-tolerated, effective, and safe alternative to traditional liver tissue sampling. Endo-hepatology is a conceptually attractive option where comprehensive diagnostic testing can be performed in a single outpatient visit after a traditional basic work-up. Although the context is favorable, and the various innovations have been validated in expert centers, we still sense a reluctance on the part of hepatologists to modify their practices and more systematically integrate the experience acquired in EUS into the development and treatment of liver pathologies. It is therefore up to us to convince not only endoscopists but also hepatologists of the validity of these innovations, by integrating our conferences into their congresses and in the training of young hepatologists who must have access to our echo-endoscopy rooms. A snapshot of current practice in Europe will be presented at the meeting. |
