EUS-guided Vascular Intervention: feasibility and safety of EUS-guided coil deployment with sclerotherapy for esophageal / isolated gastric varices | |||

[Background] Recently, We have developed the EUS-guided coil deployment with sclerotherapy (EUS-CS) for isolated gastric varices (iGVs) to eradicate not only iGVs but also feeders. Moreover, we applied EUS-CS for one case Giant tree-shaped esophageal varices. The aim of this study is to evaluate the safety, feasibility, and efficacy of EUS-CS. [Methods] EUS-CS for iGVs procedure is the following: 1) puncture iGVs using a 19G EUS-FNA needle; 2) deployment of coils until disappearance of blood flow in coil deployed iGV ; 3) inject a sclerosant (containing the contrast medium, 5% ethanolamine oleate: EO) until the feeders are imaged; 4) confirm the loss of blood flow by color Doppler. We investigated the technical and clinical success rates, accidental symptoms, and long-term advers event of EUS-CS conducted for 19 untreated iGVs cases. [Results] Technical and clinical success rates of EUS-CS for iGVs were both 16/19 (84%). The clinical success rate was 100% in 16 cases(technical success).Among the 16 technically successful cases, there was 1 recurrence (6.3%). There was no adverse event during the procedure or the observation period. [Conclusion] We demonstrated the safety and feasibility of EUS-CS under ultrasound and fluoroscopic guidance. |
