Development of a cost-effective closure method for mucosal defect after endoscopic resection

Background:
Endoscopic mucosal defect closure has considerable attention. Endoscopic hand suturing, MANTIS clip, and Reopenable-clip over the line method (ROLM) are recent trends, and their usefulness has been reported. However, their high cost and difficulty makes them problematic. Herein, we report a cost-effective closure method called side hole method (SHM).
Methods:
SHM uses an absorbable nylon thread (2-0) and EZ clips, where the thread pulled outside the scope through the side hole of the transparent hood, instead of through the forceps channel. We retrospectively collected cases of SHM from December 2023 to March 2024 and examined the utility and safety.
Results:
SHM was performed on 10 gastric tumors (SHM-G) and 6 colorectal tumors (SHM-C) after endoscopic submucosal dissection (ESD), with the complete procedure success rate of 100% (16/16). In SHM-G, the median resection size was 42(25-83)mm, resection time 36(12-110)minutes, closure time 27(16-56)minutes, number of clips used 21(17-38). In SHM-C, 51(33-58)mm,61(45-110)minutes, 42(22-55)minutes, 23(13-39), respectively. Post-ESD bleeding was observed one patient with SHM-G. No adverse events owing to SHM were observed.
Conclusions:
SHM can be performed safely with a high success rate. The advantages of this method are inexpensive, the device does not interfere with thread, and the direction of thread can be adjusted by the position of the side hole. We would like to report on the day, including the accumulated cases and video clips.
