Problems of new concept of fatty liver disease in Japan | |||

The Japanese Society of Gastroenterology and the Japanese Society of Hepatology have accepted these new concept of fatty liver diseases. Already, several researchers demonstrated that 99% of Japanese NAFLD met MASLD criteria, and MASLD prognosis should be considered equivalent to prognosis of NAFLD. However, there are some problems. Factors and thresholds related to cardiometabolic syndrome of MASLD between Japan and overseas need to be carefully considered, especially the thresholds for BMI and waist circumference. In Asia, the cut-off value for BMI is 23 kg/m2. In Japanese criteria of metabolic syndrome waist circumference is completely different, female threshold of waist circumference is bigger than male that. And cut off value of HLD-cholesterol and in female is different. Now theses problems were discussing in Guideline committee. And steatosis has not been observed in advanced ALD, particularly in Japan. Japanese pathologist proposed alcoholic hepatic fibrosis without steatosis as an independent type of ALD. In the future, it will be necessary to discuss whether all ALDs should be included in the SLD. In Japan, hepatitis viruses will be almost completely controlled, and SLD accounts for most cases of liver cirrhosis and cancer. Elucidation of the pathogenesis and treatment of SLD is a major challenge for the future. |
