EUS guided biliary drainage with Virtual Extended Reality (XR) technology | |||

Endoscopic ultrasound-guided biliary drainage (EUS-BD) has become a popular alternative for cases in which transpapillary biliary drainage is difficult, such as duodenal stenosis and postoperative gastric cases. However, puncturing and exploring the intrahepatic bile duct, which runs in a complicated tortuous fashion, can be challenging using preoperative images of CT and MRCP, as well as intraoperative two-dimensional (2D) images of EUS and fluoroscopy. Thus, it would be ideal to have a device that could confirm the bile duct route with a three-dimensional (3D) device in real time during the procedure. Virtual Extended Reality (XR) technology is rapidly becoming popular in the medical field. We have introduced this XR technology to project a 3D image of the bile duct onto the lenses of the operator and assistant wearing a head-mounted display during EUS-BD, which is useful for understanding the complicated bile duct route from various angles in real time. In this time, we report several cases in which the XR technique was useful for advanced guidewires for complicated bile duct route in the hilar region and for severe stricture of the bile ducts. A 3D cholangiogram may make it easier for some to understand the biliary anatomy than a 2D image. This is an innovative technology that allows EUS-BD to be safely performed. |
