Evolving field of submucosal endoscopy: Exploring derivatives of peroral endoscopic myotomy

"Submucosal endoscopy," also known as "Third space endoscopy," encompasses a diverse range of endoscopic techniques aiming to facilitate safe access to the submucosal and muscular layers, as well as deeper cavities within the gastrointestinal tract. This emerging field represents a novel therapeutic strategy, offering advantages over traditional surgical treatments, including minimal invasiveness and reduced risk of complications.
Performed within the "third space," which lies between the mucosal and muscular layers of the gastrointestinal wall, these procedures offer innovative approaches to managing various gastrointestinal conditions. Notably, peroral endoscopic myotomy (POEM) has demonstrated both safety and effectiveness in treating achalasia and other esophageal motility disorders. With clinical experience surpassing 3500 cases of POEM in our institution, it has been established as the standard of care. Expanding on this success, the submucosal tunneling technique has given rise to successful derivatives of submucosal endoscopies, including peroral endoscopic tumor resection (POET) for subepithelial lesions, gastric peroral endoscopic myotomy (GPOEM) for gastroparesis, Zenker's POEM (ZPOEM) for hypopharyngeal diverticula, and diverticular POEM (DPOEM) for esophageal diverticula. With over 70 cases of these derivative procedures performed at our center, clinical outcomes have been favorable, with no severe adverse events reported.
This video presentation offers a comprehensive overview of the indications and practical considerations for these minimally invasive procedures, providing valuable insights into their application in clinical practice and highlighting the evolving landscape of submucosal endoscopy.
