Current method for the prevention of stenosis after circumferential esophageal ESD-Stepwise ESD-

Introduction
The prevention of stenosis after circumferential esophageal ESD is not yet determined although various managements such as steroid injection, oral administration and application of polyglycolic acid sheets etc. have been reported.
Aim
The aim of this study is to investigate the usefulness of stepwise ESD for the prevention of stenosis after circumferential esophageal ESD.
Patients and Methods
Sixty-two patients (AC 16 and SCC 46) with esophageal cancer treated by circumferential ESD from 2006 to 2022 at our hospital were enrolled in this retrospective study. The patients were divided into 2 groups (55 patients in en-bloc (EB) group and 7 patients in stepwise (SW) group). The number of balloon dilatation (EBD) and complication etc. were investigated for each group.
Result
1. The median number of EBD was 5 (0-33) in EB group and 0 (0-0) times in SW group, respectively (p=0.0001).
2. The long axis mucosal defect was 8 (5-15) and 6 (3-9) cm, respectively (p=0.0004).
3. The length of resected specimen was 68 (29-150) and 56 (27-92) mm, respectively (p=0.0007).
4. Complication; There were no perforation and bleeding required blood transfusion in each group. And one patient in EB group had a perforation during EBD, and could be cured with conservative treatment without surgery.
Conclusion
Stepwise ESD is one of option for the prevention of stenosis after circumferential esophageal ESD.
