Metal stent placement for difficult malignant enteral obstruction using balloon-assisted endoscope

Background: Gastrointestinal stenting has become a prevalent palliative treatment for malignant gastrointestinal obstruction. With the advancements in chemotherapy extending patient survival, there is an increasing need for endoscopic techniques to manage complex conditions like afferent loop syndrome. Balloon-assisted endoscopy offers a safer method for stenting the third and fourth portions of the duodenum, the right colon, and the afferent loop than conventional endoscopic techniques.
Methods: This study retrospectively examined consecutive patients who underwent gastrointestinal stenting for malignant obstructions using a short double-balloon endoscope (short DBE) (EI-580BT; Fujifilm) at our hospital.
Results: From April 2022 to March 2024, ten patients underwent gastrointestinal stenting using the short DBE. The primary malignancies were pancreatic cancer in three, recurrent biliary tract cancer in two, colon cancer in two, gastric cancer in one, small intestine cancer in one, and ovarian cancer in one. Gastrointestinal stents were placed in the duodenum in three, afferent loop in three, ileocecum in one, transverse colon in one, descending colon in one, and sigmoid colon in one. The technical success rate was 100%. There was no bleeding, perforation following stent placement, or stent dislocation/migration during the follow-up period. Clinical response and success were achieved in all patients, with no necessity for reintervention due to symptomatic gastrointestinal obstruction.
Conclusion: Short DBE for gastrointestinal stenting is a safe and effective approach to managing difficult malignant gastrointestinal obstructions.
