Therapeutic strategies for lower GI bleeding | |||

Lower gastrointestinal (LGI) bleeding is a common and often challenging clinical condition that requires prompt and effective management. Its incidence is increasing due to the aging population and the rising prevalence of comorbidities that heighten the risk, as well as the growing use of antithrombotic agents. This presentation aims to provide a comprehensive overview of the current epidemiology of LGI bleeding and its therapeutic strategies, with a focus on recent advancements and evidence-based practices. The presentation will cover the common etiologies and pathophysiology of LGI bleeding, including causes such as diverticulosis, angiodysplasia, inflammatory bowel disease, and colorectal malignancies, as well as iatrogenic causes like polypectomy-related complications and radiation proctitis. The initial assessment and diagnostic approaches, particularly the accurate localization of the bleeding source through techniques such as colonoscopy, angiography, and advanced imaging modalities, will be discussed. Therapeutic strategies will be reviewed, starting with endoscopic interventions such as thermal coagulation, mechanical hemostasis, and injection therapy. The role of new endoscopic technologies, including over-the-scope clips and endoscopic band ligation, will also be introduced, along with their indications and effectiveness in controlling bleeding. Additionally, the presentation will address the peri-procedural management of patients on antithrombotic agents, a growing population in clinical practice. This will include the challenges and strategies for balancing the risks of bleeding and thrombosis. Emerging pharmacologic agents, such as hemostatic powders and procoagulants, will also be discussed in the context of acute management of LGI bleeding. |
