RESEARCH:
Quality of surgery in total mesorectal excision (TME), abdomino-perineal excision and colon cancer surgery demonstrating the importance of the circumferential margin, the grading of planes of surgery achieved, the variation between surgeons and the extent of mesocolic excision and the benefit of preoperative treatment on reducing positive margins.
Clinical Trials. Led the pathology in many national and international trials of colorectal cancer both surgical e.g. Classicc, Rolarr, Enrol, Trec; oncological e.g. Quasar, Focus 1-4, Piccolo, FOxTROT and radiotherapy Axis, CR07 etc..
Molecular pathology. As well as bowel cancer trials screening for molecular abnormalities including ras and B-raf mutations, DNA mismatch repair genes, amphiregulin and epiregulin for anti EGFr agents, Her2 amplification, Her3 etc... first single cell DNA fingerprinting, cystic fibrosis population screening, helicobacter pylori testing
Digital pathology and artificial intelligence. Created an extensive digital archive including 38 UK and international clinical trials www.virtualpathology.leeds.ac.uk.
Microbiome and bowel cancer. Co-I CRUK OPTIMISTICC Grand Challenge £21M. Played a role in identification of Colibactin causing mutations in APC gene, its causation of SBS88 and ID18 DNA signatures and its frequent finding in stool in a western population. Its characterisation in human CRC. The importance of the cancer microbiome in GIT tract cancers but less so outside the GI tract. Value if the microbiome in screening with Faecal Occult Blood and immunochemical testing.
