疾患マネージメントは医療環境の発展やエビデンスの蓄積により絶えず変化している.慢性膵炎においても,早期慢性膵炎の疾患概念と診断基準が明確化され,生活習慣指導を基盤とした医療介入が可能となった.内科的治療では,疼痛や脂肪便の有無に応じた栄養療法,鎮痛薬の投与,高力価膵酵素補充療法が主軸となっている.さらに内視鏡治療では,ERCPによる経乳頭的膵石除去や膵管ステント留置に加え,経口膵管鏡下膵石破砕術やEUS関連処置が盛んに行われている.一方で,内科的治療から外科治療への移行のタイミング,膵機能の温存を考慮した内視鏡治療や外科治療の適応,経過観察中の膵癌のスクリーニングの有用性などはいまだ十分なコンセンサスが得られていない.本シンポジウムでは,変遷する慢性膵炎診療の現状を整理し,残された課題と今後の展望を共有することを目的とする.幅広い演題応募を期待している.

Management strategies for diseases are constantly evolving with advances in the medical environment and the accumulation of clinical evidence. In chronic pancreatitis as well, the disease concept and diagnostic criteria for early chronic pancreatitis have been established, enabling medical intervention based on lifestyle guidance. In medical treatment of the disease, nutritional therapy, administration of analgesics, and pancreatic enzyme replacement therapy are primarily implemented according to the presence or absence of pain syndrome or steatorrhea. Regarding endotherapy, in addition to transpapillary pancreatic stone removal and pancreatic duct stenting via ERCP, peroral pancreatoscopy-guided lithotripsy and EUS-guided interventions are being actively performed. However, several key points remain controversial: the optimal timing for transitioning from medical to surgical treatment, the indications for endoscopic and surgical interventions with consideration of pancreatic function preservation, and the utility of screening for pancreatic cancer during follow-up. This symposium aims to review the current state of the evolving management of chronic pancreatitis, share the remaining challenges, and discuss future prospects. We welcome a wide range of abstract submissions.
