急性膵炎においてはしばしば後期合併症対策が問題となるが，Interventional EUSの進歩と共に，安全かつ効果的な内視鏡治療が確立されてきた．特に， 壊死性膵炎後に形成されるWalled-off necrosis（WON）は時に生命への危険をもたらす病態であるが，近年ではEUS下経消化管的ドレナージおよび内視鏡的ネクロセクトミーによる良好な治療成績が報告されている．しかし，ドレナージ介入時期や手技（デバイスの選択），経皮的・外科的治療の適応，手技に伴う偶発症対策，補助療法など検討すべき課題は残されている．また，合併する膵管断裂症候群（DPDS）の診断・治療についても討論すべき課題は多い．本シンポジウムでは，急性膵炎後に発生する諸問題に対する内視鏡の役割について，その現状と課題を皆様と共有し，今後の新たな展開について討論できればと考えている．多くの演題を期待する．

Late complications, especially walled-off necrosis (WON), are frequently encountered in patients with acute pancreatitis. In recent years, favorable treatment outcomes have been reported with EUS-guided trans gastrointestinal drainage and endoscopic necrosectomy. However, issues remain to be addressed, such as the timing and technique of drainage intervention, the appropriateness of percutaneous and surgical treatment, prevention of procedural complications, and adjuvant therapy. Furthermore, several issues remain to be discussed regarding the diagnosis and management of the associated disconnected pancreatic duct syndrome. At this symposium, the moderators aim to discuss the role of endoscopy in managing various problems that arise after acute pancreatitis and to share future perspectives with all participants.
