Pelvic recurrence of rectal cancer remains one of the most challenging issues in surgical treatment of rectal cancer. Both Japanese and Western guidelines emphasize that complete surgical resection offers the greatest chance of improving prognosis. In recent years, multidisciplinary treatment combining radiotherapy and systemic chemotherapy has also been frequently employed, with an emphasis on balancing curative potential and patient quality of life in treatment selection. Advances such as laparoscopic and robotic surgery, carbon ion radiotherapy, and novel systemic agents have further expanded therapeutic options; however, standardized strategies in Asia are still in the process of being established. This panel discussion will provide an opportunity for experts from various disciplines to share the latest practices and challenges, and to explore optimal treatment frameworks from an international perspective.

直腸癌の骨盤内再発は，本邦および欧米のガイドラインにおいて外科的完全切除が最も予後改善に寄与する戦略と位置づけられている．近年では，放射線治療や全身化学療法を組み合わせた集学的治療を施行されることも多く，治療選択にあたっては根治性と患者QOLの両立が重視される．近年は腹腔鏡やロボット支援手術，重粒子線治療，新規薬剤の導入などにより治療の選択肢は拡大しているが，アジアにおける標準治療は確立途上にあるのが現状である．本パネルディスカッションでは，各分野のエキスパートによる最新の実臨床と課題を共有し，国際的視野から最適な治療体系を探る機会を提供したい．
